B12 above 750 pg/mL

Bone-turnover markers
CTX
CTX reflects osteoclast activity and bone resorption.
Values above 350 pg/mL may suggest excess bone breakdown.
One U.S. testing option is Labcorp 500089.

avoid biotin and collagen for 48 hours
avoid strenuous exercise for 24 hours
P1NP
P1NP reflects bone building.
A practical target range is 20–100 µg/L.

P1NP to CTX ratio
This ratio can be especially useful.
A working range of 100–300 is one way to frame it.
A ratio above 150 may suggest bone building is winning.
There is one caveat.
If nutrient intake is too low, both markers may look low and falsely reassuring.
Urine DPD
Urine DPD is another bone-turnover marker.
It rises when osteoclast activity is high.

Hormonal and endocrine context
Estradiol can also matter when hormone replacement is being used.
Practical context:
an estradiol level above 50 pg/mL is often used as a target when HRT is being used for bone support
if HRT starts more than 10 years after menopause or after age 60, lower estradiol targets may be safer
pre-existing plaque raises the need for caution when estradiol rises above 35 pg/mL
The higher cardiovascular risk has mainly been described in women who begin HRT after age 60.
Specialty root-cause testing notes
24-hour urine calcium
A result below 300 mg/24 h may suggest a renal calcium leak.
That means the kidneys may be spilling calcium instead of reabsorbing it.
Get a baseline test after 3 days without calcium supplements, then repeat after supplementation if needed.
24-hour oxalate
A value above 30 mg/24 h may suggest oxalate-related renal leak or kidney-stone risk.
