Emerging immune-oncology relevance of silymarin and milk thistle

Silymarin is the active extract of Milk Thistle, and this page explores how Milk Thistle / Silymarin may influence the tumour immune microenvironment.

Overview

Beyond direct anti-tumour effects, Silymarin / Milk Thistle may also modulate immune signalling around the tumour. This is an emerging but important area of research.

PD-L1 suppression

A 2024 colorectal cancer study reported that silymarin suppressed PD-L1 expression in colorectal cancer cells.

That matters because PD-L1 is one of the immune-checkpoint signals tumours use to reduce immune attack.

CD8+ T-cell activation

The same work also reported increased inflammatory CD8+ T-cell populations in tumour-bearing mice.

That combination is notable: lower PD-L1 signalling together with stronger CD8+ T-cell presence points toward a potentially useful immune-supportive direction.

Broader immune modulation

  • Silymarin may influence cytokine patterns in the tumour microenvironment

  • It may help shift immune behaviour in an anti-tumour direction in selected contexts

  • Human combination data with checkpoint inhibitors is still limited

Practical takeaway

Milk Thistle / Silymarin is not a proven immunotherapy substitute. It is, however, a compound with growing immune-oncology relevance that may eventually prove useful as a complementary adjunct.
