Curcumin's activation of the Nrf2 pathway and why it matters clinically

NRF2 impact: activator

Curcumin is a well-characterised activator of the Nrf2 pathway.

In normal cells, this can be beneficial.

  • Activates Nrf2/HO-1/NQO1 antioxidant defence pathways

  • Upregulates phase II detoxification enzymes

  • Supports cytoprotection in healthy tissue

Important caution

Nrf2 is not always straightforward in cancer biology.

Some tumours use high Nrf2 activity as a survival mechanism. In those contexts, strong Nrf2 activation may be less desirable, especially around treatment.

That does not make curcumin inappropriate. It does mean tumour biology and timing matter.

Practical takeaway

Curcumin's Nrf2 activity is one reason for its favourable safety profile. It is also one reason clinicians may want to individualise timing and combinations in active treatment settings.
